WHAT SHOULD I DO IN CASE OF EMERGENCY?
In
the event of a severe situation, life threatening
problem, or one in which you are in doubt, go
immediately to the Emergency Room of the nearest
hospital. Once there, ask someone at the Emergency
Room to contact your doctor. If the situation
is not extremely severe, but one where you wish
contact with your doctor, telephone us or come
to the office to speak to the Triage Nurse.

HOW
SHOULD I PROCEED WITH PRESCRIPTION REFILLS?
All prescription refills should be requested
during normal office hours and at least two
days before your prescription runs out. Medications
which are considered controlled must be written
by provider and given to patient. These cannot
be called into any pharmacy.

MAY
I LEARN MORE ABOUT THE FEES AND PAYMENTS?
We make every effort to keep down the cost of
your medical care. You can help by paying any
appropriate co-payments, deductibles, or other
charges if you do not heave health coverage
for services at the completion of each visit.
You may also qualify for a federally approved
program of patient care to be paid for on a
sliding scale. Proof of income and family size
is required every six months. Ask our receptionist
for details of this program intended to improve
your access to quality patient care. If necessary,
we can assist you in developing a payment plan,
or by submitting information to your insurance
company.
Please bring your insurance card medical card,
City Welfare card or pay stub for your appointment.
If paying cash for your visit we will we will
charge you according to our sliding fee scale,
which is based on family income and family size.
Payment is due at the time of service. Your
cooperation with the required information will
assist the receptionist in registering you in
a more efficient and quicker manner.

INSURANCE
& OTHER PAYERS:
The Health Center participates in General Assistance
(City Welfare), Medicaid programs, and also
accepts most major insurances. We DO NOT require
a deductible if you have Medicare B.
